“Making the Move”
Future Transitions is a service available to youth and young adults, with a Developmental Disability, between the ages of 13 and 28. This service is designed to help young people plan for their future, and assist with a smooth transition from school into an adult oriented activity.
Youth’s interests are explored, skills and abilities are assessed and plans are made utilizing Person Centred Planning or Transition Planning held through their school.
The Transition Coordinator is a support to the youth, their families and the school and a link between Community agencies. Supports and resources are based on the individual’s needs and interests.
The key goal is for each individual to make informed choices that will lead to successful transitions from school to community based activities; community living and work/ volunteer activities.
Connecting and building community networks could include:
- Work/employment preparation
- Daily living skills
- Personal safety awareness
- Social skills
- Further education
- Volunteer experiences; and/or
- Community recreation and leisure.
