Independent Living Unique to Each Person’s Needs

Individualized support for people who live on their own or with a room mate or partner. The plan will be developed with the professional team, family and individual to create a person centred plan to meet the needs and personal goals of the individual to promote autonomy and independence. Supports can be arranged to assist as often or as little as they are needed, depending on current needs or events and stages of life. Support can be provided on a daily, weekly, or monthly basis.

How We Help:

Teaching activities of daily living such as menu planning, grocery shopping, laundry

Understanding budgeting and helping develop a financial plan to pay bills and rent

$

Understanding health problems, assistance with doctor or dentist appointments and follow up to medical recommendations

$

Monitoring and/or administration of medications

$

Finding and securing housing as well as maintaining tenant/landlord relationships

$

Connecting to other community supports, services and recreational opportunities

$

Communication and relationships with peers, partners, family and neighbours

$

Parenting, family planning
