This position supports individuals as outlined through the Support Plan process and through discussions at team meetings. This position will work 32 hours per week: Mon, Wed, Thurs and Fri with flexibility for evening and weekend planning. Provides direct care as required. Assists with program planning and implementation. Promotes independence and utilizes community resources to facilitate integration into the community to full potential. Acts as a liaison between internal and external contacts. Pr0motes positive family contact and represents the Agency favourably. Works with team members in the best interest of the clients. This position is a flexible contract with two week’s notice given to shorten or extend the contract.
- Human Services Diploma ie. DSW, SSW, CYW or equivalent preferred
- Current First Aid/CPR certificate
- Current NCI
