The plan will be developed with the professional team, family and individual to create a person centred plan to meet the needs and personal goals of the individual to promote autonomy and independence. Supports can be arranged to assist as often or as little as they are needed, depending on current needs or events and stages of life. Support can be provided on a daily, weekly, or monthly basis.
